ADVANCE HEALTH CARE DIRECTIVE
An advance health care directive allows a person to choose who makes health care decisions for them in the event they are unable to make those decisions themselves. It also sets forth a person's wishes regarding end-of-life care, organ donations, cremation vs. burial and any other specific wishes.

TRUST TRANSFER DEED
In order to make your Trust effective, you must transfer ownership of your assets into the Trust. We do some of this through a Trust Transfer Deed (other assets like IRA accounts and bank accounts also need transferred). Our offices prepares the necessary Preliminary Change of Ownership Report, Transfer Tax Affidavit and Trust Transfer Deed before submitting it all the Recorder's Office for filing.
